The study is a pragmatic multi-centre, two arm, parallel design, superiority, randomised controlled clinical study. The study will be conducted in 286 patients aged 14 years and above, with a proximal tear of the ACL diagnosed by MRI, from at least 20 secondary care sites with acute knee pathways in place. Participants will be individually randomised (1:1) intraoperatively following arthroscopic assessment, to receive either ACL repair or ACL reconstruction.

Each participant will be followed up for twenty-four months. The study will determine if ACL repair is superior to ACL reconstruction at 24 months post-surgery. This will be measured by a questionnaire relating to the knee called the Knee injury and Osteoarthritis Outcome Score (KOOS)-4.
